with you when you
come for your first

Your First Visit |

Upon entering our
office our
receptionist will
greet you and
welcome you as a
member of our
family. We will
request the New
Patient Health
History Form you
filled out online if
you didn't get it
faxed to us, or you
can fill one out in
the office.
The paperwork
provides us with
general information
about you and your
condition. |

Next you will have a
consultation with
Dr. Rick Andreasen
to discuss your
health-related
problems, concerns,
as well as potential
treatment options.
Your first visit is
designed for Dr.
Andreasen to learn
more about you, your
condition and your
expectations to
determine whether
chiropractic care
will you meet your
goals. |

Dr. Andreasen will
conduct a physical
examination of the
problem areas, and
may need to take
some x-rays. If
treatment is
appropriate on the
first visit Dr.
Andreasen will start
you on your road to
health and wellness,
or will schedule a
date for your
treatment to begin. |
